Terms and conditions for 4x Month Membership - 3 Month commitment
STROLLER STRIDES MONTHLY MEMBERSHIP AGREEMENT
This contract made between the client and FIT4MOM Cleveland West Side is a 3 month agreement and except as provided in the initial 3 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend in person or virtual Stroller Strides, Strides 360, and Body Boost classes up to 4 times per month. Up to 2 unused classes per month will roll over as long as you have an active membership. There is a one time $40 cancellation fee if you cancel the membership before the 3 month period is over.AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Cleveland West Side to initiate a charge to the credit card number entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM franchise owner.
I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Stroller Strides to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.
CANCELLATION POLICY
If you cancel after a three month commitment is completed, there is no fee to cancel. If you cancel early, there is a $40 one time fee. To cancel this membership, the member must contact the FIT4MOM franchise owner at kellykitsis@fit4mom.com at least one week prior to their next payment due date to request a cancellation. Any requests received after the 7 days will not go into effect until the following month. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the Cleveland West Side franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee.
HOLD POLICY
Memberships may be held a maximum of 4 months per hold period. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period. Note that hold fees will be waived for maternity leave.
INITIAL 3-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the third (3rd) calendar day after you have signed this agreement. To cancel this agreement, written notice must be received by FIT4MOM Cleveland West Side within the initial three days. You are responsible for paying for the classes already attended.
